چکیده

Proximal functional kyphosis (PJK) is a common complication (26-39%) following multilevel instrumented fusion for spinal deformities. It is defined as a focal segmental kyphosis of more than 10 degrees between upper instrumented vertebra (UIV) and 2 levels cranial to UIV (UIV+2); and a focal increase in kyphosis of more than 10 degrees between UIV and UIV+2 [1]. Although the clinical impact of this radiological phenomenon was initially unclear, recent literature has clearly demonstrated it to be the most common cause for revision surgical interventions following spinal deformity correction surgeries [2]. The exact cause underlying this complication has still remained elusive. Nevertheless, multiple clinical and radiological factors have been shown to negatively influence the alignment of spinal segments immediately proximal to the fused vertebrae [3].

Selection of proximal fusion level for degenerative scoliosis and the entailing proximal-related late complications.

OBJECTIVE To determine the optimal selection of proximal fusion level for degenerative scoliosis (DS) and investigate the long-term proximal-related complications. METHODS Profiles of 95 consecutive patients with DS who underwent posterior long instrumented fusion were analyzed retrospectively. Perioperative parameters were reviewed stratified into 3 groups according to the relationship betwe...

Preventing proximal junctional failure in long segmental instrumented cases of adult degenerative scoliosis using a multilevel stabilization screw technique

BACKGROUND The authors sought to demonstrate the safety and effectiveness of the multilevel stabilization screw (MLSS) technique in decreasing the incidence of proximal junctional failure in long segmental instrumented fusions for adult degenerative scoliosis. METHODS Institutional review board approval was obtained and all patients with adult spinal deformity who underwent the MLSS technique...

Proximal Junctional Kyphosis in Early Onset Scoliosis

In adults and adolescents, proximal junctional failures above deformity constructs have been well established with different risk factors identified; however, the incidence, nature and clinical significance of this kyphosis remains unclear. Several reports in the literature have shed light on PJK in adults and adolescents with variable definition, incidence, prevalence, acuteness, risk factors,...

The effect of distraction-based growth-friendly spinal instrumentation on growth in early-onset scoliosis.

The present study assessed the advantages and disadvantages of growth-friendly spinal instrumentation surgery for early-onset scoliosis in 17 patients who underwent this surgery with a minimum 2-year follow-up. The mean number of lengthening procedures was three, initial age at which surgery was performed was 108.1 ± 30.2 months, and follow-up duration was 40.6 ± 16.6 months. Spinal height (T1-...
